1. I'm aware that in order to apply & be referred to work I must have already turned 18 years old & obtained a high school degree or equivalent.

I understand that if called to work by the Union, any false statements shall be considered sufficient cause for dismissal from the Extra List & referral system. Local 5 is authorized to investigate previous employment & education records.
3. If called for referral by the Hiring Hall, a service charge of 5% of my gross earnings shall be paid to Local 5 through payroll deduction.
4. I hereby authorize a $0.50 (Fifty cent) deduction from my wages for a voluntary charitable contribution by me to the Performing Arts Technical Scholarship Fund (PATS Fund.)
